33. Unannounced Visit: Broken and Missing Fixtures Copy

Investigation Findings
Licensing Program Analysts made an unannounced visit to the New Way Group Home #2 for the purpose of conducting a Case Management Inspection to address physical plan violations observed during today’s visit.

Upon arrival, LPAs toured the exterior and interior of the facility. LPA inspected the rooms and found bedroom #2 and bedroom #3 were missing screen protectors for the windows. Per staff, the screen protectors are in the garage because the clients take them off all the time. In addition, the blinds in the front entrance by the garage window are broken and need to be replaced.

A review of facility’s personnel summary on 04/11/18 indicates that all staff in the facility have Child Abuse Index checks, and have DOJ/FBI criminal record clearances.

The following deficiency is cited per California Code of Regulations, Title 22 Regulations, Division 6, chapters 1 & 5.

Exit interview was conducted with the Facility Administrator. Appeal Rights were issued and a copy of this report was left at the facility.
