Fingerprint Clearance
All employees, interns must have DOJ and FBI clearance prior to working on the premises. Complete the Request For Livescan Lic 9163 form to begin the fingerprint process. Complete Section 5.
San Fernando Police Dept is a livescan fingerprint site and is a three-minute walk from Life Skills. Take your completed form to a LiveScan Site to be fingerprinted. You must be clear prior to your start date at Life Skills. Clearance usually takes about one to two weeks. You can check the status of your clearance here.

First AID and CPR Certificate
All staff must have a current First Aid CPR Certificate from live training prior to working at Life Skills. Many of our staff obtain their First Aid Certification from. Patty For CPR at 818.470.2037
You can obtain First Aid CPR certification from any approved provider.
We will reimburse you for a LIVE CPR FIRST AID course.

Medical Screening (include clearance LIC 503 with documents by US MAIL
All staff must obtain medical clearance prior to working at Life Skills.
The medical evaluation must include a TB test and must be completed by a Physician on The Health Screening Form LIC 503.
You can use any licensed medical provider you wish.
You may have a chest X-Ray in lieu of the Mantoux TB Screening Test. Many of our staff have had their medical clearance completed here:

Other Agency Clearance
What if I am already clear through DHS or Consumer Affairs?
Clearance from any another agency e.g. Board of Psychology or a law enforcement agency is not transferable. Agencies that are transferable are under the regulation of Community Care Licensing e.g. Foster Care or Residential Care for the Elderly.
Even if you have a current Community Care Licensing clearance you still need to obtain fingerprint clearance in order to be associated with Life Skills. The transfer process is not feasible since DPSS does not provide a record of the transfer.
