All applicants must complete LIC 508 and declare whether they have ever been convicted of a crime other than a minor traffic violation regardless of whether the individual was granted a pardon for the conviction or received an expungement pursuant to Penal Code 1203.4 or the individual’s record was sealed as a result of a court order. Marijuana-related offenses covered by the marijuana reform legislation codified at Health and Safety Code sections 11361.5 and 11361.7 need not be disclosed.
On LIC 508 the applicant must disclose: reckless driving, drunk driving convictions, assault and battery.
