Investigation Findings: Licensing Program Analyst arrived at the home to discuss Complaint #18-AR-20180312125203. During the complaint investigation, LPA toured the home and noted several deficiencies. Some of the deficiencies were repeat violations from LPA’s annual visit on 6/23/17.
